Q 1.
From what Google images has turned up it could be a Jolana Kramer...

Q2.
or a Kramer Beretta III.

Q 3.
Yes. The tremolo bridge is an Ibanez ZR2 bridge tremolo with a ZPS3 spring system.

The ZR2 (Zero Resistance) bridge with the ZPS3 spring system returns you to "zero" ” center point ” after each use of the tremolo arm, even in the case of string breakage! No more wondering if you're back in tune or worse, knowing you're not! One more way to provide more dive-bombing freedom with less hassle.

I have one of these ZR2 tremolos on my Iabnez s5470 TBK Prestige guitar. It holds up to incessant wankery very well and the ball bearing system is pretty slick. Though Ibanez has abandoned the ZR2 tremolo in favour of their Edge series.

Cheers!

B III was a Jolana creation.
This plate clearly has a S prefix which indicates Schaller Jolana made. Bound non-American script HS as well.
